2017年度暑期菲华学生厦门、泉州学中文夏令营

Application for CKS students interested to join the Xiamen/Quanzhou Summer Study Tour 2017 organized by the Tan Yan Kee Foundation and the Philippine Education Research Center is now open.  

 

资格：

a)华校10岁以上华裔在读生；

b)身心健康，品行良好；

c)中英文各科、各阶段成绩均为及格以上；

d)由学生在读学校推荐。

Interested CKS Students must

a)      at least be 10 years old or above at the start of the study tour,

b)      be physically fit, of good moral character as shown in the conduct grade.

c)      have passing marks in all subjects for both English and Chinese instructions at all periodic during School Year 2015-2016, and

d)      be endorsed by CKS College to participate in the said event.

 

营期：

第一批：2017年3月30日至5月23日        

第二批：2017年3月31日至5月24日

Study Tour Dates:

Group 1 – March 30 to May 23, 2017

Group 2 – March 31 to May 24, 2017

 

课程：语言课，文化课（音乐、绘画、手工、武术等），旅游

Activities would include Chinese language classes, cultural lessons (music, drawing, handcrafts, martial arts, etc.), and tours.

 

Documents to be submitted:

a)      Application Form duly filled up, with the applicant’s signature on the bottom right corner and signature of the parent or guardian and the seal of the school on the bottom left corner.

b)      Certified photocopy of the student’s SY2015-2016 Chinese and English instruction report cards. Passing grades for ALL Chinese and English subjects in ANY PERIODIC is required.

c)      Original Passport, if not Chinese citizen. Expiry date should not be earlier than November 2017. Please note in your application form if passport is for renewal. Philippine passport holders are advised to leave a photocopy with him/her for DSWD Travel Clearance application.

* Submit also a photocopy of the applicant’s passport bio-page and last page for visa application.

* The original copy must reach the organizers not later than January 14, 2017.

Photocopy of the Passport, if Chinese citizen. Expiry date of the passport should not be earlier than November 2017.

d)      Three pieces 2” x 2” photos white background.

e)      Visa Application Form if not Chinese citizen. Fill up all items with the applicant’s signature at the last page.

Chinese passport holders must present his/her Alien Certificate of Registration Identity Card (ACR I-CARD) on the orientation day to his/her designated teacher-in-charge.

f)        Waiver Form duly signed by parents or guardian

g)      Participation fee amounting to USD460.

* All payments should be made thru Philippine National Bank BILLS PAYMENT or BDO cash deposit in branches nationwide. Participants are advised to keep a duplicate copy.

* Applicant should write his/her English name, and school name on the payment slip/deposit slip.

* Attach one original copy of the machine validated payment slip/deposit slip.

h)   DSWD Clearance if Filipino minors (under 18 years of age). Participants should secure his or her own clearance.

需要交的报名材料：

a) 报名表：按表格内容要求填写完整，表格下方必须有参加者父母（监护人）和参加者本人签名，并由就读学校盖章。切勿遗漏！

b) 中英文成绩单：提交2015—2016学年度的成绩单复印件，复印后由学校盖公章方为有效。成绩要求中英文每科、每阶段均须为及格以上。

c) 护照：非中国籍的须交护照正本，护照有效期须为2017年11月及以后时间。如护照在办理中或更新中，请在报名表上注明。否则，因护照有效期不够的原因被拒签，后果自负。如果在此期间需用护照的，可以先交复印件，但正本必须在2017年1月14日以前交到菲律宾华教中心。

   因办签证需要，办理报名手续时，请同时将护照的首页和最后一页复印交来。否则不予批准。

中国籍者交护照复印件即可，但是护照的有效期也必须为2017年11月及以后时间。

d) 照片：2×2的白底照片3张。

e) 签证表及出入境手续：非中国籍的须填签证表，要把表格的内容全部填完，并签名。非菲籍的学生出入境手续必须自己办理，出发前召开团务会的时候必须交给带队老师。

 f)  契约书：根据菲律宾有关法律规定，必须签署健康与安全风险责任追究弃权契约书。

g) 费用：请按照《厦门/泉州学中文夏令营汇款须知》办理。每人须缴交460美元。其中60美元为代收杂费。

h) 18岁以下的菲律宾籍学生必须自行到社会福利部（DSWD），准备自己去办DSWD清单用。

 

报名材料及银行收据需于2016年10月14交給葉啟明主任，缺一不可，否则不予批准。
